To expedite the process for you as a new client, you can download the new client forms below, fill them out, and bring them to us on the day of your child’s initial evaluation.

Please remember to arrive at least 15 minutes prior to your appointment time.  Also, be sure to bring the physician’s prescription for therapy and a copy of your child’s insurance card.
